Instructions
During the conference:
Non-US Citizens: make a copy of your white or green I-94/I-94W
arrival/departure record (the form you fill out on the airplane).
Obtain and keep your hotel receipts. Your receipt
must show that you have paid your bill and must indicate the
method of payment (e.g., credit card).
After the conference:
Please send all of the items below:
Your original receipts:
Airline tickets: send in your receipt showing payment
by credit card or check. If paying by other means (including cash),
enclose a signed statement that you have paid the airfare
and that you will not be reimbursed for the airfare from another
source.
Hotels: send in a receipt showing that you have paid your
bill with a credit card. If paying by other means
(cash, travelers checks, etc.) enclose a signed statement
explaining how you have paid and that you will not be reimbursed
from another source.
If you have no receipt that states that you have paid and
that shows the method of payment, it is helpful to enclose a
photocopy of your credit card statement showing the payment.
